Health Requirements for Student Registration Physical Exam: Required for students entering kindergarten, sixth, and ninth grades, as well as all new students. Dental Exam: Students entering kindergarten, second, sixth, and ninth grades must have a dental exam or a dental waiver form. Eye Exam: Kindergarten students must have an eye exam or an eye exam waiver. Obtaining Forms: These forms can most likely be obtained from your doctor/dentist's office.
