ATTENTION PARENTS/GUARDIANS OF WARREN CUD #205 STUDENTS REGISTRATION DATES/TIMES Tuesday, August 9 – registration 9:00 am-12:00 pm and 12:30 pm - 6:00 pm Wednesday, August 10 - registration 9:00 am – 12:00 pm and 12:30 pm – 5:00 pm 6^th grade orientation 6:00 pm Registration will be held in the administration wing of the school. K-8 parents will enter through the east door & 9-12 parents will enter through the main office door. Both offices will be open at the above times for your convenience. Students entering kindergarten, sixth, and ninth as well as all new students in our district must have a physical to be turned in at registration. Students entering kindergarten, second, and sixth must have a dental examination form or dental examination waiver form and all kindergarten students must have an eye exam or eye exam waiver to be turned in at registration.

Please read and have your child read the appropriate handbook. Print the acknowledgement form, have both you and your child sign it, and bring it to registration with you. If your Jr/Sr high student is participating in a sport this year, please include the IHSA acknowledgement as well.
